In the early morning hours of July 23, 1982. Filming was underway at Indian Dunes, a 600 acre ranch in Valencia, California for a scene set in Vietnam for the first segment of Twilight Zone: The Movie. A pyrotechnic went off causing a low flying Bell UH-1 to spin out of control, with the tail rotor to detach from the tail. Below, actor Vic Morrow was carrying two illegally hired children from the exploding village and waded across a shallow river. One of them fell into the water, Morrow attempted to grab the child when both the actor and one of the children were decapitated whilst the other was crushed by the copter's right landing skid.
